結紮法(endoscopic band ligation : EBL)
内視鏡先端にフードを装着し、出血店を周囲粘膜ごとフードないに吸引し、ゴム製のO-リングでポリープ状に結紮する方法。結紮による物理的な血流の遮断、結紮部位の潰瘍形成、線維化を起こすことで止血をはかる。一般的には内視鏡的静脈瘤結紮術(endoscopic variceal ligation :EVL)として、食道静脈瘤の治療に用いられる。
食道静脈瘤他、Dieulafoy潰瘍、Mallory-Weiss症候群。GAVE、DAVE、憩室出血などの非静脈瘤出血に対しても有効との報告がある。
The method of endoscopic band ligation (EBL) involves attaching a hood to the tip of an endoscope, suctioning the bleeding site along with the surrounding mucosa into the hood, and ligating it into a polyp-like structure using a rubber O-ring. This technique achieves hemostasis by physically blocking blood flow, causing ulceration at the ligated site, and promoting fibrosis. It is commonly used as endoscopic variceal ligation (EVL) for the treatment of esophageal varices. EBL has also been reported to be effective for non-variceal bleeding conditions such as Dieulafoy ulcers, Mallory-Weiss syndrome, gastric antral vascular ectasia (GAVE), Dieulafoy-like lesions (DAVE), and diverticular bleeding, among others.
