Have you ever heard about mother and child health book?

In Japan, expectant mothers are given a booklet for recording their health condition during pregnancy and important information regarding childbirth as well as your baby's health until they grown up. This notebook is also used to record of the child’s vaccinations and growth until they turn around 6 years old.
Your doctor will give you a pregnancy notification form once they hear your baby's heartbeat sound through trans vaginal ultrasound around 6-8 weeks pregnant.
Then, bring the form to your city hall or local municipal office with your identification and the registration card of your hospital.
