まとめ：
今回は月に一度のHeartRhythm, Europace, EJHF, JLHTを含むので多めです。
一番のインパクトはNEJMの虚血性心疾患を対象としたFirst lineとしてのVTアブレーション。ガイドラインに影響がでそう。個人的にはHeartRhythmのEffective CRTの研究に注目。有効なCRTペーシング後増やす必要があるが、この機能は現状ではMetronicのみ。Holter心電図では評価困難。

緩和ケアを救急ユニットで開始しても死亡率、ホスピス入所率、在宅医療率などを改善しない。はやすぎる緩和ケアは効果がないと考えられているがサポートになる論文。

PFA後のセカンドアブレーションでは44％PV rreconnectionがあったことに対するコメント。これは手技者や施設ばらつきがありそうだが、意外と初回PVI成功率は低い？？

薬物治療との比較のおいて1年以内に施行されたCryoの有効性は証明されているが、実際に1年以内と、1年以降に違いを調査した研究。早期Cryoは再発率の低さに関連。

Ineffective cardiac resynchronization pacing is associated with poor outcomes in a nationwide cohort analysis
Effective CRTの重要性を示唆する論文。当たり前といえば当たり前だがEffective CRT率が高い症例ほど生存率が高い。Effective CRTは80％以上を目指した方がよいか？この研究からは25％がEffective CRTが75％程度のよう。他メーカーでどう対応するかが問題。
