#3 再生医療　北京の出産時に胎盤&臍帯の保管するかどうか検討/ Placenta & Umbilical code “To store or not to store”

北京で外国人として胎盤と臍帯を保管したい場合

  • 概要
    私は現在妊娠37週目で、初めての出産ですが、北京で出産を予定しています。
    通院しているユナイテッドファミリーでは直接は提供していないため、会社の紹介だけしてもらいました。ただ、紹介しているのみでユナイテッドが直接運営はしてないとのこと。なので通訳はつけられず、詳細はその会社に電話して確認が必要でした。

    私たちの夫婦の結論は、北京で外国人として胎盤と臍帯保管はハードルが結構高そうで不確実性が多いのでしないことにしましたが、備忘録として書きました。
    また他に検討したい方や中国•北京に長期的に滞在が確定している人のためにご参考までにです。

  • 注意点
    英語のWEBはあるものの、電話や問い合わせは中国語のみ。契約書なども中国語のみの想定
    色々な選択肢、パッケージ価格があるそうだか、胎盤保管だけなら安価でも、実際にいざ使用したいときには輸送費や手続きで別途費用がかかる（国外に輸送する場合は税関手続きや特別な輸送手段が必要になることが予想される）

胎盤保管の価格について　(2024/10月時点の電話確認)

• 料金プラン(外国人向け)：
1. 胎盤+臍帯：49,800 RMB （約110万円で、30年間保管）
2. 胎盤+臍帯血：49,800 RMB（約110万円で30年間保管）
※先方からのおすすめは(1)の胎盤と臍帯保管とのこと、理由が臍帯血のみでは血液量が少なく、培養が難しいため。

• 手続き：
1. 保管施設の見学は予約が必要（2日前までに）。施設見学は希望者のみで不要の場合は特に予約不要
2. WeChatで情報を受け取れる。
• 胎盤と臍帯の受け取り方法：ユナイテッドファミリーの病院側に事前連絡し、出産時に病院で適切な処置を依頼。
• 使用時：日本の医師から何に必要なのかの資料などを発行してもらい、日本の法律に基づき郵送可能か確認後、輸送手配。その際に、輸送費や手数料は別途かかるが、現時点では回答できないとのこと。
• 支払い方法：銀行口座振込またはWeChat決済。

メリット


胎盤と臍帯の保管の意義は再生医療で子どもの将来の病気の可能性のため
• 胎盤: 多能性幹細胞が豊富で、臓器や組織の再生治療、自己免疫疾患、神経修復などに活用の可能性がある。
• 臍帯: 臍帯血には造血幹細胞、臍帯組織には間葉系幹細胞が含まれ、血液疾患や組織修復の治療に活用可能。家族間での利用適性が高く、再生医療の発展でさらに活用の幅が広がる可能性がある。
両方を保管する意義: 胎盤と臍帯は異なる幹細胞を含み、それぞれ補完的な役割を果たすため、保管により多様な治療に対応できる。


北京でのハードル


北京で胎盤と臍帯を保管し、私たちが万が一、日本で子どものために使用したい場合に際に想定されるハードルは、法律・規制、医療機関の対応、保管環境の信頼性、費用、国際間の調整など、事前に確認・準備する必要があると想定。
なので、結果的に関心はあるものの私たちは実施しない方向で決めた。
もし多数、日本の実績があれば考えたのかもしれないけど、それがわからなかったため。

【ハードルと考えられるもの】
1. 法律および規制の制約

• 輸送に関する規制：日本の法律で幹細胞や臓器に該当する物質の輸送には厳しい制限があるため、胎盤や臍帯の持ち込みが許可されるか確認が必要。
• 国際輸送における生物材料の輸送規制（生体試料の取り扱い基準）もクリアする必要がある。
• 輸入許可：日本への輸入には厚生労働省などの許可が必要となる場合がある。

2. 医療機関の対応

• 受け入れ体制：日本国内の医療機関が、北京で保管された胎盤・臍帯の利用を受け入れるかどうか。
• 保管施設の品質基準や採取手順が、日本の医療基準に適合しているか確認が必要。
• 医療機関の協力：必要な際に日本の医師が適切な書類（診断書、使用指示書）を発行してくれるかが重要。

3. 保管環境の信頼性

• 施設の国際基準適合：北京の保管施設が、日本を含む国際基準（例：ISO、AABBなど）を満たしているか。※ホームページでは、AABBは2010年に取得と書いてはある。
• 長期保管の安全性：30年間の保管期間中、保管環境が適切に維持される保証があるか。会社として2009年にスタート。まだそもそも30年は経っておらず、今後30年安定して提供できるのかは、わからないので、個々の判断。

4. 費用の負担

• 輸送費用：温度管理が必要な特殊輸送のため、輸送コストが高額になる可能性。
• 手続き費用：書類作成、許可申請、検査などの手続きに追加の費用が発生。

5. 使用時の技術的課題

• 適合性の確認：再生医療で使用する際、保存した幹細胞が目的に適合するか（量や品質の問題）。
• 培養技術や医療技術の進化：保存期間中に医療技術が進化し、保存した胎盤・臍帯が最新技術で活用できなくなる可能性。

What we decided
We inquired about placenta and umbilical cord storage through United Family Hospital in Beijing. They dont offer services directly but they introduced us to a company that offers this service. However, they only provide referrals and do not directly operate the service. Therefore, interpreters are not available, and all details must be confirmed by directly contacting the company.

After considering the matter, we, as foreigners living in Beijing, have decided not to proceed with storing the placenta and umbilical cord. However, we are sharing this information for those who may be interested in exploring this option. Perphas for those who know who will live in Beijing or within China for a while, it could be more feasible.

key points to consider
• Language Barrier: Although the company provides an English website, inquiries and contracts are handled exclusively in Chinese.
• Costs Beyond Storage: While there are various pricing options, including affordable plans for placenta storage only, additional costs such as transport and procedures will arise when the materials need to be used. For international transport, customs clearance and specialized shipping methods are likely required.

Price for Placenta Storage (As of October 2024 - Based on Phone Inquiry) for Foreigners:

1. Placenta + Umbilical Cord: RMB 49,800 (~JPY 1.1 million) for 30 years of storage.
2. Placenta + Umbilical Cord Blood: RMB 49,800 (~JPY 1.1 million) for 30 years of storage.
• The company recommends the first option (Placenta + Umbilical Cord) because umbilical cord blood alone contains insufficient blood for culturing.
• Procedures:
1. Facility tours are available by appointment (at least two days in advance). A tour is optional and not mandatory for storage.
2. Information is provided via WeChat.
• Placenta and Umbilical Cord Collection:
• Notify United Family Hospital in advance and request proper handling during childbirth.
• Usage:
• Obtain necessary documentation from a Japanese physician detailing the purpose.
• Verify compliance with Japanese regulations for shipment, then arrange transport. Shipping costs and fees are additional, but exact amounts are currently unclear.
• Payment Options: Bank transfer or WeChat payment.

Challenges of Storing Placenta and Umbilical Cord in Beijing as Foreigners


If we were to store these materials in Beijing and later use them in Japan, the following hurdles would need to be addressed
1. Legal and Regulatory Restrictions
2. Medical Facility Coordination
　　3. Reliability of the Storage such as Environment(Compliance with International Standards). According to the company’s website, AABB certification was obtained in 2010 yet will have to consider with Japan side and their acceptance.
      4. Cost Burden for Transport Costs,Administrative Fees
     5. Technical Challenges at the Time of Use

So, we decided that we will not be storing it for our baby.
It would have helped if they had list of counties in the past they have sucuessfuly done this but wasnt sure so as much as we were intrested we decided not to.
