アメリカ引越し　結核スクリーニング

小学校入学時に必要な書類で結核スクリーニングの項目あり。
トラベルクリニックに確認すると
小児の結核スクリーニングができないかもしれないと言われ
かかりつけ小児科に聞くと英文書類出せないと・・・

どうすればいいんだと悩んで
色々なクリニックに問い合わせした結果、
世田谷区にある小児科でなんと英文作成してくれると
わかりました。

少し遠いので私の運転が不安でしたが
当日、無事に到着。

結核スクリーニングは２回、病院に行く必要あり。
１回目注射、２回目注射後の腫れの大きさを調べる

注射の腫れは痛々しい感じに見えますが
子供達はかゆいとだけ言っていました。

この腫れを調べ、陰性か陽性の書類を英文で作成してもらうのが流れ。とりあえず入学書類は、あと健康診断のみ。それは現地でやろうかと思っています。
何をすればいいのか全く不明。


Registration

Beginning October 1, 2022, you can register your student online! Whether you want to enroll your student in their school of residence or choice into a different school or charter, you will need to complete the standard online registration form with your district of residence.
You will need the following items to register your student in their school of residence:

  • Student’s Birth Certificate

  • Proof of Residency

  • Photo ID of Parent/Guardian

  • Custody Documentation (if applicable)

  • Health Records (as required in Delaware Education Health and Safety Regulations):

    • Proof of Immunizations

    • Proof of a Physical Exam (within the last two years)

    • Proof of TB Screening

    • Proof of Lead Test (if applicable)

  • Previous Student Records (if applicable)

    • Report Card

    • Transcripts

    • Withdrawal Sheet

    • Individualized Education Plan (IEP)

    • 504 Plan
